One service that we offer is Post-Hospital Support. With experienced Regional Nurses working with hospital staff, if you are discharged from hospital following a stay for surgery or illness, we can help you transition back into your own home.
We offer all rehabilitative services and support as well as up to 24-hour care in-home – so if you want to recuperate, at home, you can do so safely and with support.
This service is also available following day surgery if you live alone – we can offer transport to and from the hospital, can stay overnight to keep an eye out for any issues and check on any clinical needs.
This type of specialised care is not usually available in the community, and recovery at home is preferable in most cases following a hospital stay.
If you need ongoing support in the home, then our Live in Care option might be appropriate.
This is a much more affordable option than residential care and allows you to remain in the comfort of your home with the support you need to remain independent.
For 8 hours in a 24-hour period, you will have active support and companionship for your needs. That might include cooking and cleaning, transport to social events or appointments, medication and continence care or more hands-on nursing – depending on your individual requirements. For the other 16 hours, the carer will be in the house and can assist when needed; for example, night-time toileting and emergencies.
If you have high aged care needs and require more support, then our 24-Hour Care might be most appropriate. A regular team of carers will provide 24-hour cover for you, to ensure that there is always someone on hand capable of supporting you. This option is also suitable for palliative care.
If your needs are more complex – such as dementia care or palliative care, our team are trained and ready to help.
When it comes to Dementia Care, we know that this term is used to describe the symptoms of a wide range of diseases which cause a progressive decline in a person’s cognitive abilities. When this is happening, remaining at home can help in many ways – keeping the client in familiar surroundings is orientating, promotes self-esteem and supports independence. Our service recognises that no two patients are alike, and that is why our dementia care services are tailored to suit the individual.
For Palliative Care, we have supported many patients with their decision to remain at home. This is a personal, painful and challenging time for the patient and the family, so we work with GPs, specialists and Community Palliative Care teams to provide options that facilitate the wishes of our clients.
