Post hospital support, often referred to as ‘post discharge home care’ or ‘transition care’ is the nursing and support given after a hospital stay while in the comfort of your own home. The goal is to actively help nurse you back to better health and assist with regaining independence and confidence as soon as possible.
We offer both short-term and long-term specialised transition care focused on healing and monitoring your recovery after you return home from hospital. Our registered nurses can assist with wound care, medication, catheter care, pain management and much more.

Private nursing at home means you can enjoy your bed and creature comforts with the convenience of a nurse who comes to you as often as you require.
