What is medication assistance?
Medication assistance is support to take the right medication at the right time - prompting, supervision and help with blister packs or Webster-paks, recorded against your care plan and your pharmacist's instructions.
Can DANS help reduce medication mistakes?
Yes. Missed doses, double-ups and confusion between old and new prescriptions are among the most common risks for people managing several medications at home. Regular medication support reduces those risks and flags problems to your nurse or GP early.
Is medication assistance suitable after hospital discharge?
Yes. Prescriptions often change at discharge, and the first weeks home are when mistakes happen. DANS can provide medication support as part of post-hospital care at home, short-term or ongoing.
Do you manage Webster-paks at home?
Yes. DANS can assist with Webster-paks and other pharmacy-prepared medication packs as part of an approved medication support plan. Depending on your needs, a trained support worker or registered nurse can prompt, supervise or assist with medication administration, check the pack is being used correctly and report any concerns to your family, pharmacy or treating team.
